Maria qualified as Registered General Nurse in January 1991. Following her qualification she worked as a staff nurse on both medical and surgical wards covering a range of specialisms including gastroenterology, neurology, older person care and surgical procedures.
In 1997 Maria became a community nurse undertaking a variety of clinical nursing procedures along with patient assessment, care planning, monitoring and evaluation.
She obtained my BSc (Hons) Advanced Healthcare Practice in 2005 which enabled her to undertake leadership and management of other team members alongside working closely with other members of the multi-disciplinary team including GPs, social services, care agencies and the voluntary sector.
In 2006 she undertook the Long Term Conditions module at Plymouth University and then became a community matron.
Between the years of 2007 and 2008 she undertook further academic MSc modules at Plymouth University to advance my knowledge and skills. These included:
- Applied Clinical Skills for healthcare
- Integrated Biosciences and Applied Pharmacology
- Legal Principles in Health Care Practice
- Clinical Decision Making
- Consultation, Principles and Practice of Data Analysis
- Research and Project Design coursework.
In 2008 Maria achieved the Extended/Supplementary Prescribing module.
The role of Community Matron is the lead in the development of primary care for older people with multiple long term conditions who have a high risk of hospital admission. This is a proactive role focusing on ways to improve patient health care, empowering patients and their families to make informed decisions regarding their disease(s). She worked at an advanced level of autonomous practice including taking managed risks and clinical responsibility, for example allowing a patient to stay at home rather than being admitted to hospital. Maria worked collaboratively with other professionals, carers and families to gain a deeper understanding of all aspects of patient’s physical, emotional and social situation. This includes detailed history taking, physical examination, diagnosis and treatment planning which enables individualised patient needs to be identified and met.
As Care Manager with In Caring Hands, Maria is involved in the assessment, care planning, implementation and evaluation of complex needs service users, ensuring that the staff with the right skill set are appropriately matched to each service user.
The company is divided into two areas of care being Community Social Care and Learning Disabilities. Both are overseen by Maria German who has appointed Emma O’Reilly to oversee the Community Social Care team and Luke Lawrence to oversee the Learning Disability team based on their knowledge, skills and experience.
